Memory Matters exists to support brain health during times of transition and change.
Whether an individual is living independently or adjusting to assisted living, our purpose is to strengthen cognitive function, support adaptive routines, and promote continued engagement.
By providing early guidance and intentional brain health support, Memory Matters helps families make informed decisions while preserving independence and dignity.
Memory Matters was founded to support individuals and families who want to be proactive about brain health and independence.
Our work is guided by dementia-informed training, person-centered care principles, and real-world experience supporting older adults through cognitive change.
Rather than focusing on tasks or supervision, Memory Matters focuses on understanding the brain, adapting routines, and supporting dignity at every stage.
Memory Matters takes a proactive, structured approach to brain health and cognitive support.
We begin with a personalized brain health assessment to understand strengths, challenges, and daily routines. From there, we guide individuals and families in building adaptive, brain-healthy routines designed to support cognition and prolong independence.
Our focus is not on doing more — but on doing what matters most, early and intentionally.

The Brain Health Assessment is a comprehensive, dementia-informed evaluation designed to understand how an individual’s brain is functioning over time — not just in a single visit.
This assessment typically takes up to five visits to complete, allowing Memory Matters to observe cognition, routines, and engagement across different moments and settings. One of these visits is conducted with family members only, creating space for open conversation, education, and planning without placing pressure on the individual.
Cognitive changes are rarely clear in a single moment.
A multi-visit assessment provides a more accurate picture, helping families make informed decisions and build a plan that supports independence, dignity, and brain health early.
➡️ This comprehensive assessment is included in the 3-2-1 Brain Wellness Coaching program and is strongly recommended before ongoing visits.
Our Assessments start with the family caregiver. Schedule an assessment by calling us at (704) 870-7204 or text ASSESSMENT to 980-525-9084.
