Our comprehensive assessment:
- Identifies the stages of the dementia and the symptoms that may coorelate with those stages based on your loved ones behaviors, patterns and abilities.
- Includes four 1.5 hour visits with the person showing cognitive decline. These visits will include brain wellness activities, assessing the abilities to do ADLs
Our comprehensive assessment:
- Identifies the stages of the dementia and the symptoms that may coorelate with those stages based on your loved ones behaviors, patterns and abilities.
- Includes four 1.5 hour visits with the person showing cognitive decline. These visits will include brain wellness activities, assessing the abilities to do ADLs (dressing, bathing, toileting and eating) and iADLS (cleaining, shopping, paying bills, and cooking) and will also test the ability to learn new things and remember what they learned.
- 1st visit is with the family caregiver/s. As many as are able to attend the better. We will get to know the family dynamics, the financial overview, the legal overview, and any concerns or observations that will be pertinent to know before the visits.
- Last visit includes a comprensive summary based on our observations and your input, a clear picture of where their behaviors line up with each stage of dementia and a followup meeting to see What's Next.

After a clear picture is identified of you family dynamics, family abilities to care, and what the financial, legal, and medical standings are, you can choose between the following on-going (but short-term) services:
- Caregiver Support and Navigation Text-based Coaching
- Family Caregiver Liaison and Advocacy
- Transition to a new living envirome

- Transition to a new living enviroment consulting
- Memory Matters program (typically 3-6 months)
- Out of area Family Caregiver support when family member with dementia is local to Winston-Salem and surrounding areas
